Household-use Declaration
This declaration is the promise you make when activating your private Health vault. It is separate from accepting the Private Vault Processing Terms.
What do you promise?
[ ] I confirm that, for every Health profile, I will choose either that the profile is about me or that I have the person's permission or appropriate legal authority to manage the information for personal or household purposes. I will enter only necessary information, respect each person's privacy and rights, and stop updating and delete a profile if the permission or authority I rely on ends. I will not use Health for professional healthcare, employment, insurance, surveillance, harassment, advertising, research or unlawful purposes.
What choice do you make for each profile?
Before Planique creates a Health profile, you must select exactly one of these statements:
- This profile is about me.
- I confirm that I have this person's permission or appropriate legal authority to manage this information.
The second statement is your declaration that the represented person can validly give the permission you rely on, or that you have legal authority appropriate to the circumstances. Planique does not ask you to upload proof during ordinary profile creation. A custom label or optional relationship label is for organisation only and is not evidence of identity, permission, guardianship or authority.
What does this mean?
By activating Health, I also understand that:
- Health is a personal organiser, not medical advice or an official medical record;
- a custom or relationship label is for organisation only and does not prove identity or legal authority;
- Planique does not routinely verify represented people or require their separate accounts, documents or consent forms;
- permission must come from a person who can validly give it; otherwise, I must rely on legal authority appropriate to the circumstances;
- I must not keep adding or changing information after the permission or legal authority I rely on ends, and I must delete the affected profile;
- Planique may investigate a credible privacy or misuse complaint and may proportionately restrict, require deletion, delete or suspend affected content;
- only my authenticated account can access and manage the profiles;
- this declaration does not prove my identity, relationship, guardianship, legal authority or another person's consent; and
- withdrawing this declaration requires Health deactivation and deletes Health profiles and all information saved in Health, including cycle history and estimates, reminders and Calendar entries, from active systems.
What does Planique record?
When you activate Health, Planique records the current declaration version and action text, the language shown, your choice, timestamp and relevant app metadata using an immutable legal event.
When you create a profile, Planique separately records whether you selected “about me” or “permission or appropriate legal authority”, together with this declaration version, the legal-content and interface languages, timestamp and limited app metadata, in an immutable profile-authorization event. It does not record proof documents or treat an optional relationship label as evidence.
If Planique publishes a material replacement of this declaration that requires a new response, accepting it reaffirms the recorded basis for every profile you continue to keep in Health. Before continuing, you must delete any profile for which the recorded basis is no longer accurate.
These records show what you declared. They do not prove your identity, permission, guardianship or authority and do not mean that you consented on behalf of every person represented in a profile.
